30 year old female with back pain and multiple joints pain

CASE PRESENTATION:

Patient came with c/o

Back pain since 3 years 

Multiple joint pain since 3 years 

HOPI:

Pt was apparently asymptomatic 3 years back then she developed lower back pain which gradually extended upto neck region and shoulders 

She also complained of multiple joint pain (involving  DIP ,PIP,MCP,WRIST,ELBOW,SHOULDER,HIP,KNEE, ANKLE,MTP  of both Upper and Lower limbs)

Increased pain in right lower limb compared to left lower limb

Pain aggravating on walking for longer duration,doing house hold works

Pain relieved on taking rest,stretching and massage

No c/o morning stifness of joints

No h/o Fever,headache 

No h/o nausea, vomiting 

No h/o constipation or loose stools 

PAST HISTORY:

Not a k/c/o HTN/DM

Not a k/c/o TB/Asthma/Epilepsy/CAD/CVD 

PERSONAL HISTORY:

Diet : mixed

Appetite : Normal 

Sleep : Adequate 

Bowel and Bladder movements : Regular 

No Addictions 

On Examination :

Patient is conscious, coherent and cooperative.

No pallor, icterus, cyanosis, clubbing, lymphadenopathy, edema.

VITALS:

TEMP. : 98.4 F

PR : 80 BPM

RR : 14 CPM

BP : 110/70 MM HG

ON EXAMINATION:
